脑重要结构的辨认及勾画
1.颞叶勾画(图1-23)
放疗靶区危及器官颞叶的勾画较传统的颞叶为大,即其包括了传统的颞叶和岛叶,其具体边界:
上方:外侧裂上方。
下方:颅中窝底部。
前方:颞骨和外侧裂、蝶骨大翼之间。
后方:颞骨岩部、小脑幕、枕前切迹之间。
外侧:颞骨。
内侧:海绵窦、蝶窦、蝶鞍、外侧裂,包括海马旁回和海马回。

2.海马结构
海马结构与人类的记忆密切相关,而且对放疗较为敏感,因此如果海马照射,会出现人的认知障碍,而且临床研究已经明确,脑转移癌少有发生于海马结构者,因此对脑转移癌行全脑放疗,海马的防护就显得尤为重要。
下面介绍的为海马勾画原则(参考RTOG网站提供标准)(图1-24)。
海马辨认及勾画原则:
MRI扫面层厚应不超过1.5mm;
T1平扫横断面上勾画;
海马结构位于灰质,因此T1平扫表现为低信号区域,不能勾画高信号的白质区;
主要参考结构为侧脑室,一般从下向上勾画,最下层面以侧脑室下角开始出现的层面勾画。

A1~A3:侧脑室下角为海马结构寻找的标记,侧脑室下角内侧的灰质结构即为海马(红线显示,下图等同);B1~B3:随着层面上移,海马结构逐渐移向后上方;C1~C3:依次向上勾画;D1~D3:侧脑室下角,注意海马与其前方的杏仁核(蓝线范围)和内侧的海马旁回(黄线范围)鉴别,后者也为灰质,MRI T1像同样表现为低信号;E1~E3:海马位于侧脑室下角的后外侧,但相应层面应注意勿把其前方同样低信号的杏仁核和内侧的海马旁回勾画在内;F1~F3:海马位于脑桥环池的外方;G1~G3:海马位于脑桥环池的外下方;H1~H3:继续向上勾画,海马位于脑桥环池的外下方;I1~I3:继续向上勾画,海马的辨认以逐渐出现的侧脑室后角为参考;J1~J3:海马的辨认以清晰的侧脑室后角为参考;K1~K3:继续向上勾画,海马逐渐由侧脑室后角的内方移向前内方;L1~L3:海马位于侧脑室后角的前内方;M1~M2:最后层面的海马结构;N1~N2:这些层面已经没有海马结构
图1-25为MRI矢状面及冠状面所显示的海马结构。红线范围内为具体海马结构,绿线为海马外扩5mm作为海马危及器官的保护区域。
